Who is eligible for care?

UA Local 412 Members and their dependents enrolled in their insurance plan are eligible for care. 

What kind of visits are available?

In-clinic, Telehealth and Video visits.

How much does it cost? 

There is no cost to use the clinic; there are no copays and no deductibles. This is a free benefit for eligible members and their dependents.  

Can I get a same day or next day appointment?

Members and their dependents have access to same day or next day appointments for acute needs (illness, injury, etc). You can schedule online, call the office to schedule or send us a message in Spruce. 

What if I have an urgent medical need after hours? 

No problem! You and your dependents have free access to Revive, a 24/7 telehealth urgent care and mental health platform for all of your urgent after-hours needs. Ask us for more details. If you are experiencing an emergency, please call 911 or go to your nearest emergency room.  

Is my information private?

Yes. Your employer does not see your medical records.

Do I need insurance?

You don't need to worry about networks or surprise billing we make access clear and simple.
